How do they estimate a fold change in the following situation (normalized counts already:

treated1 treated2 untreated1 untreated2
10           10           0.8             0.6 

or if all untreated groups have zero counts.

What kind of pseudo-counts is used?

Take a look at the DESeq2 paper for an intuitive description of the fold change estimate. It's not a pseudocount based method, but a Bayesian method, which uses the information from the experiment to determine the best estimate when there is little information (eg. low counts, which can vary a lot).
